Services are performed by the doctor that will provide the most coverage for the patient, while minimizing out-of-pocket costs.

In most cases, there are some fees that are NOT covered by insurance but are required in order to enter the WLIA program. Due to variances in individual insurance plans, your out-of-pocket expenses may vary and your exact costs will be discussed with you in greater detail at your initial consultation.
Dr. DeBarros and WLIA accept credit or debit cards, certified or cashier’s checks, and cash. We do not accept personal checks. We require your balance be paid 2 weeks prior to your scheduled surgery date.
It is your right as a patient to request information or have questions answered regarding fee or payment policies. If you have any questions or problems regarding your bill from EVSC, please contact our Business Office at (480) 829-6100.

In the year you finance your weight loss procedure, you may be eligible to receive thousands of dollars back in potential tax savings – think of this as a discount on your procedure! IRS Publication 502 states you can deduct only your medical and dental expenses when they total more than 7.5 percent of your adjusted gross income (see p. 2 of publication 502). Adjusted gross income is your total income less a few very specific deductions. Medical expenses may include fees paid to doctors, payments for hospital services, prescription drug costs, physician co-pays and the cost of participating in a weight-loss program for a specific disease, including obesity.
Speak with your tax advisor or see IRS publication 502 for details.
